Direct Care Staff (DCS)

FUNCTIONS:
This position provides personal care, instruction, guidance, mentoring, companionship, and support according to the individual’s person-centered plan/individual support plan (ISP). The position provides a level of care that enhances the health, safety, dignity, and contentment of participant served.

RESPONSIBILITIES:

  • Promote the mission and vision of This Generation Cares.
  • Ensure compliance and implementation of TGC’s policies and procedures.
  • Train participants ISP/IPP before providing service. (to identify specifics such as food allergies, behavioral issues, emergency contact #, etc.)
  • Assist participants with personal hygiene and other activities of daily living through modeling, coaching, and structuring activities.
  • Maintain a pleasant, tidy, sanitary, and safe living environment for participants.
  • Manage crisis situations learned in agency-approved training and participant’s crisis safety plans.
  • Transport participants to scheduled appointments, communal, and recreational activities.
  • Successfully complete medication administration training to administer prescribed medication(s) and document it accurately.
  • Review and complete required daily documentation accurately in logs and communication books.
  • Communicate appropriately with participants, staff, and others.
  • Communicate and resolve problems and concerns with residential manager; participates in staff meetings.
  • Ensure effective communication with participants, families, and other individuals involved in the provision of supports for consumer.
  • Participate in team and service meetings for participants as necessary.
  • Participate in projects and work groups as assigned.
  • Complete 24 hours of relevant training annually.
  • Perform other duties, as necessary to engage participants with leisure activities, such as arts & crafts, and games.

QUALIFICATIONS:
Candidate must be 18 years of age and have an equivalent of a high school diploma. 1 year experience with mental intellectual disability (mental retardation) preferred. Must be able to physically lift and move participants. Must have good interpersonal and organizational skills, as well as strong written and verbal communication skills. Must meet act 33 and 34 requirements and have a valid license to drive in the state of PA, and properly insured vehicle.

Community Support Staff (CSS)

FUNCTION:
The purpose of the Community Support staff is to provide individuals with a quality level of care thought supportive services in the home and community that contributes to the individual’s independence. Assisting with the plans and goals outlined in the individual’s ISP.

RESPONSIBILITES:

  • Promote the mission and vision of This Generation Cares.
  • Ensure compliance and implementation of TGC’s policies and procedures.
  • Assist with personal hygiene, dining, ambulating, transportation, and behavioral support.
  • Meet with direct program specialist weekly for supervision.
  • Must document on progress notes all activities and duties performed with the individual as outlined in the individuals ISP.
  • Complete and submit progress notes.
  • Complete documentation as requested by the individual’s family and Supports Coordinators.
  • Current and valid PA Driver’s license.

Qualifications:
Candidate must be 18 years of age with a high school diploma or GED. Experience with mental intellectual disability preferred. Must be able to physically lift and move individuals if needed. Applicant must have good interpersonal and organizational skills, and strong written and verbal communication skills. Must meet act 33 and 34 requirements and have a valid license to drive in the state of PA, and properly insured vehicle.
